At Balanced Medical Solutions, we offer patient forms online so you can complete them from the convenience of your own computer and submit them directly to us through a secure forms system. Please note that you must use a computer (not a cell phone) and that you must complete it in one sitting as your work is not saved. Patient forms take approximately 15-20 minutes to complete. Due to the multiple levels of security and encryption on these forms, please allow for up to 60 seconds for the form to load and to send once completed.
